We ask that you fill out the online form by clicking on the link that was emailed to you or you will need to arrive 5 min early to your appointment to fill it out in office.
When you arrive for your scheduled appointment, please text "Here" to 319-200-8000 and wait in the large waiting room. We will come and get you when we are ready for your appointment.
Cancellations
Please provide us with 24-hour advance notice for cancellation. If you cancel or No-Call/No-Show within less than 24-hours notice you may be required to pay the cancellation fee of $25. If you reschedule more than 3 times in a row then you will be canceled and asked to reschedule when you know you can in fact make it.
Courtesy
No video recording or pictures of the private areas will be allowed!
Please make sure that you are on time for your appointment. Anyone more than 5 minutes late, for any reason, will be asked to reschedule.
